AFHSC is the central repository for deployment health assessment forms (DD2795, DD2796, and DD2900).  AFHSC does not directly collect electronic versions of the forms.  These forms are collected using customized applications that have been developed by the individual services and forwarded to AFHSC.  As of August 2007 AFHSC no longer processes paper deployment forms (DODI No. 6490.03 of August 2006 requires that all forms be submitted electronically).

AFHSC provides electronic images of these forms to Tricare on a regular basis.  These forms are available for viewing by health care providers via accounts provided through Tricare Online.
